General Attendee Registration Form

This form is divided into three parts. The first section collects some basic information about your company or organization. The second part allows you to list the people from your group who will attend the conference and select their session type and meal choice. As you add or remove attendees the fee table will be updated to reflect your choices. At least one person must be listed in section two to process your order. The last section collects your payment details.

Email addresses — Once your order has been processed we will email a copy of your reciept to the email address you provide in section one. Any email addresses provided in section two will be used to notify those attendees. It is perfectly fine if the email address you enter in section one matches one of the attendees listed in section two. In fact, this would be expected if you are the only attendee.

Refund Policy
General Attendees The original registrant may request a full or partial refund through 5pm PST on February 2, 2019. A fee of $20 will be deducted from the refund amount to cover transaction fees. Refunds will be issued to the same card used to pay for registration. Refunds can be requested by calling 1-503-222-6254 between 9AM and 4PM Monday through Friday.

Exhibitors The Official Pacific Northwest Conference Cancellation Policy states that exhibitor fee refunds will be made in full until 30 days before March 1, 2018. If you cancel within 30 days of March 1, 2018, your fees will be forfeited in full.

Sponsorships All event sponsorships are final and not eligible for a refund.
